Single individuals, domestic partners, married couples and their dependents are eligible. Dependents are eligible up to the child’s 26th birthday. Membership may be continued for a dependent over the age of 26, if the dependent is incapable of self-sustaining employment by reason of developmental or physical handicap. Parents or guardians who would like to enroll children or dependents on their own plan may do so.

The Individual/Family 1300 Plan cannot be used as a secondary or supplemental plan. If you have a primary dental plan, please contact your dental provider for options.
Most commonly performed dental procedures are listed on the Fee Schedule Any procedure not listed on the Fee Schedule will be offered at a 20% discount of the provider's usual fee. Only procedures performed at contracted offices are covered under this plan.
Membership in Individual/Family 1300 Plan become effective the same day the enrollment application and membership fee are received, although it normally takes 5 to 7 working days to process your application. You will receive your membership card shortly thereafter. If you have an emergency before receiving your membership card, call 888-900-0316 and we will verify eligibility.
